The State of Delaware offers Health Care Connection for those patients who qualify. Patients who were formerly on CHAP may apply for Health Care Connection. New patients who meet specific state guidelines for financial assistance may also qualify.


Delaware's Health Care Connection

Screening For Life Program and the services provided will not be affected by this change, as Screening For Life is an exception to Division of Public Health Policy Memorandum No. 43: Provision of Health Care Services to Person Eligible for Health Coverage.

Beebe’s bilingual coordinators screen patient's for eligibility and can guide the patient with any one of a number of medical assistance programs, including discounted services offered by Beebe Healthcare.

Health Care Connection (HCC) became effective June 24, 2015.

How to Enroll

When you enroll in this program, Health Care Connection (formerly Community Healthcare Access Program or CHAP), may enable you to receive health care at a reduced cost from participating doctors, clinics, hospitals, etc. There is no cost to enroll in this program.

If you are someone who:

  • cannot afford to pay costly medical bills
  • does not have health insurance through an employer, or does not have health insurance through the Insurance Marketplace as required
  • has been denied Medicaid coverage
  • is self-employed

To speak with a Health Care Connection (HCC formerly CHAP) Health Promotion Advocate please call 211 or 302-744-1040. Screening for Life and Health Care Connection (HCC formerly CHAP) are Programs for uninsured Delaware residents who meet specific requirements. The programs are administered by the Delaware Health Care Commission
