Beebe’s Coordinated Care Center bridges the gap between inpatient and crucial follow-up care for patients without a primary care provider. These transitional visits are vital to avoid rehospitalization and connect patients with a primary care provider for continued support. Patients are given an appointment prior to their discharge. The Center does not accept walk-ins.
