$1.75 Million Grant to Support Expansion of Beebe Coordinated Care Center

Beebe Healthcare welcomed Delaware’s Congressional Delegation to the Long Neck Health Center on Tuesday, Sept. 8, to highlight a $1.75 million Congressional Directed Spending investment that will support the expansion of Beebe’s Coordinated Care Center.

U.S. Senators Chris Coons and Lisa Blunt Rochester, along with Congresswoman Sarah McBride, joined Beebe leaders for the announcement.

Beebe launched the Coordinated Care Center in Lewes in August 2024. This Center helps support the transition between inpatient and crucial follow-up care for patients without a primary care physician or advanced practice clinician. These transitional visits are vital to avoid rehospitalization and connect patients with a primary care provider for continued support. Patients are given an appointment prior to their discharge. 

Thanks to this funding, Beebe will be able to relocate and expand the Center, increasing access to primary care, care coordination, and chronic disease management in a rapidly growing area of Sussex County. The Center is expected to move in summer 2027 after construction and renovations to the existing building on the Long Neck Health Center. 

The event included remarks from David A. Tam, MD, MBA, CPHE, FACHE, President & CEO of Beebe Healthcare; Liane Sorenson, Board Member; Kristi Zangari, DO, Medical Director of Beebe Medical Group Hospitalist Service and Chair of the Department of Medicine; and Bill Chasanov, MD, Chief Health Systems Design Officer at Beebe Healthcare.

"The Coordinated Care Center helps bridge the gap between a hospital stay and long-term health, connecting patients to primary care, care coordination, chronic disease management, and other support services that help them stay well.

"By reducing avoidable hospital readmissions and helping patients access timely follow-up care, the center can help reduce unnecessary emergency department utilization while connecting patients to the right care at the right time. This investment will allow us to expand that proven model, bringing care closer to home while advancing our commitment to value-based care focused on prevention, wellness, and better health outcomes.
